Membership Forms and School Enrollment Forms

Ready to join? Download a Membership form, fill it out, and mail it to Kadima: 12353 8th Ave. NE, Seattle, WA  98125. You do not need to be a member to enroll your child(ren) in school, but higher tuition rates apply to nonmembers. A $180 Introductory Membership level is offered to new preschool families only. If you have any questions about the forms, call Kathy at the Kadima office, (206) 547-3914. She will answer your questions or help you fill out the form.
